 Introduction

This course explores best practices in HAZOP leadership and management. You will learn about the application of the technique and how to plan and manage study programs more effectively. You will also learn how best to lead study teams to ensure maximum effectiveness and successful project execution.

Case studies will allow you to undertake a range of project team roles and provide the opportunity to guide a team through the HAZOP process.

Objective

    prepare for a HAZOP study meeting and program – including defining the scope of the study and choosing the team
    choose nodes (parts of the drawings or operation for HAZOP study)
    estimate the program requirements for the successful completion of a study
    use facilitation techniques to motivate the team and keep them on task
    avoid common problems encountered during study meetings, including challenging behaviors
    formulate the HAZOP study report
    understand the team leader’s role in the implementation of recommendations and the management of the process.

Target audience

This course is for individuals experienced in the HAZOP technique who are required to lead HAZOP studies.  They must have experience as a team member and understand the HAZOP methodology. Experience as a HAZOP study scribe would also be useful.

Outline

Day 1:

  •     Leadership responsibilities

  •     Planning a HAZOP program

  •     Node identification and time estimation

  •     Preparation of the event

  •     Recording style, skills, and development of recommendations

  •     HAZOP meeting facilitation

  •     Team dynamics

  •     Leadership through the study

  •     Development of the final report and action management

Day 2:

  • Techniques for Hazard Identification and Analysis – HAZOP

  • Where and When to Use HAZOP and the Requirements for a Successful HAZOP Study

  • Team Composition for HAZOP Studies

  • Guide Words and Process variables used for HAZOP Studies

Day 3:

  • HAZOP Team Leader / Facilitator Requirements

  • HAZOP Scribe Requirements

  • Facilitating HAZOP Studies

  • Needed Information to allow Successful HAZOP Studies

  • Review of Commercial Software used for HAZOP and Management of Change ‘MOC’

Day 4:

  • Theory Behind Fire, Explosion, and Toxic Dispersion Modelling Utilised in Quantitative Risk Assessments

  • Types of Fires and their Effects on People and Equipment

  • Types of Explosions and their Effects on People and Equipment

  • Review of Software

Day 5:

  • QRA

  • The Role of Event Tree Analysis

  • The Role of Fault Tree Analysis

  • ETA and FTA

  • Failure Data for Use in QRA’s

  • Societal Risk and Individual Risk

  • Review of Software available for Quantitative Risk Assessments
